Position Purpose:
The Nurse Practitioner (NP) is an advanced practice nurse, who utilizes advanced knowledge and clinical expertise in providing specialized health care services to pediatric patients, families, groups, and communities across the health-illness continuum. Diagnostic reasoning, advanced therapeutic interventions and education are key elements in the direct provision of care using a collaborative model. The NP is an autonomous practitioner who works in collaboration with a physician caring for both inpatient and outpatient pediatric BMT patients. The NP is accountable for making clinical judgments using independent and interdependent decision-making skills for patients undergoing autologous, allogeneic, and umbilical cord blood transplantation.
The NP utilizes his/her skills as an advanced practice nurse to care for patients across the care continuum. Currently, there are 2 NP positions that alternate inpatient and outpatient care. In collaboration with the physician, the NP has direct responsibility for the management of the medically complex BMT patients. These responsibilities include documentation in EMR, placing medical orders, consulting with other specialties, and interpreting appropriate diagnostic laboratory tests. While caring for the inpatients, the NP should or will develop an understanding of the BMT process, including types of BMT, chemotherapy administration and its side effects, complications related to BMT, and other acute complications. While caring for the outpatients, the NP should or will develop an understanding of care of the BMT patient post-transplant, including complications related to BMT, re-immunization, late effects to therapy, and collaboration of care with both medical and psychosocial disciplines.
The nurse practitioner also functions as a consultant, educator, researcher, resource and role model for patients, families, the community, and other health care providers. The nurse practitioner evaluates the quality of care rendered; analyzes and promotes system changes to enhance quality of care and organizational effectiveness/efficiency; conducts/utilizes research to promote health and improve health care delivery; serves as integral team member for FACT accreditation. The nurse practitioner assumes responsibility for his/her own professional development. Appropriate privileges are required to prescribe pharmacological and medical interventions.
Qualifications:
- Master’s degree in nursing from an accredited College or University
- Two to 5 years working as a pediatric RN; previous experience as an NP preferred
- Hematology Oncology, Critical Care, and or Blood and Marrow Transplant experience as an RN or NP preferred
Certificates, Licenses, Registrations: Registered nurse licensure to practice in the state of Michigan. State of Michigan Nurse Specialty Certification as a Nurse Practitioner.
APHON certification
Pediatric nurse practitioner, will consider Family nurse practitioner with prior Hematology Oncology or BMT experience
Expectations and Responsibilities:
Under the limited direction of the attending physician or designee,
- Works autonomously and in collaboration with a physician.
- Obtains and documents patient medical history; performs physical examinations; orders, evaluates, and monitor medical and laboratory diagnostic tests and treatment plans.
- Prepares, develops, and initiates treatment plan in consultation with attending physician including determination of need for consultation of other specialists.
- Educates and counsels patient and family members on condition, treatment plan, desired outcomes, expectations, and other related areas across the care continuum.
- Participates actively with other members of interdisciplinary team in weekly team meeting and in twice weekly meeting with Karmanos Cancer Center.
- Educates nursing staff on inpatient, outpatient, ICU units regarding care of BMT patients.
- Serves and participates in quality management program including analyzing data quality measures, FACT accreditation, and submission of data to CIBMTR.
Working Conditions:
- Occasionally lift and/or move up to 25 pounds with or without assistance.
- Able to complete duties under stress, deadlines, and while attending to multiple duties simultaneously.
- Prolonged computer related exposure, as well as sitting and standing at workstations for long periods of time.
- Noise level in the work environment is usually moderate.
- Bending, sitting, and standing frequently throughout the day.
- Occasionally travel off-site for meetings, conferences, seminars, and training.
