Function: Provides patient centered supportive & palliative care for patients in inpatient setting, working with the primary referring physician and the Supportive & Urology Pediatrics Group.
Scope: Performs clinical functions in a dynamic patient care environment through active participation with other providers.
ESSENTIAL JOB FUNCTIONS
Practices in an expanded role to provide health care to individuals, families and/or groups in a variety of settings. Acts independently or collaboratively with other health professionals in the delivery of health care services. Provides teaching, consultation, and performs research.
Conducts comprehensive symptom and psychosocial assessments as appropriate on SPC patients in the hospital setting using physical examination skills and assessment tools
Uses organizational skills to assure continuity of care across the continuum in collaboration with other disciplines through appropriate planning, education, and coordination of services
Participates in developing treatment plans with referring physicians, palliative care physicians, and other health care professionals as appropriate.
Provides education and support to individual patients and families, emphasizing care planning (both advance care planning and transitional care planning, and the promotion of comfort and quality of life
Provides symptom management expertise and psychosocial support to patients and families
Serves as a liaison between patients/families and other members of the healthcare team
Participates in the collection of patient data with appropriate documentation in patient record
Coordinates the activities of the hospital based palliative care consult team with the referring physicians and other members of the All Saints staff
Participates in meeting the educational needs of self, other professional staff, patients, and families in both the outpatient and inpatient setting. This function will include participation in the development and provision of educational programs in palliative care for other health care professionals at the local, state and national levels.
